Tattoo Consent

Tattoo consent.

Prior to each tattoo session, clients are required to read and sign this consent form.

1. Identity and Age

By signing this form, you declare and warrant that:

  • You are at least 18 years of age at the time of the appointment.
  • The identity information you provide is accurate and complete.
  • You are the same individual as the identity provided.

Valid government-issued identification (ID card, passport, or driver's license) may be required prior to the tattoo session. Devarda.ink reserves the right to refuse service without adequate proof of identity.

2. Health Declarations

You are responsible for fully and accurately disclosing all relevant health conditions. This form helps Devarda.ink assess your eligibility.

Conditions That Must Be Declared

  • Blood disorders: anemia, hemophilia, clotting disorders, or anticoagulant medications.
  • Skin conditions: eczema, psoriasis, keloid tendencies, open wounds, or active skin infections.
  • Allergies: to tattoo ink, latex, lidocaine, antiseptics, or skincare products.
  • Immune system: autoimmune conditions, HIV/AIDS, hepatitis, or immunosuppressive medications.
  • Cardiovascular disease: heart conditions, uncontrolled hypertension, or pacemaker.
  • Diabetes: uncontrolled blood sugar conditions affect healing.
  • Pregnancy or breastfeeding: currently pregnant, trying to become pregnant, or breastfeeding.
  • Medications: prescription medications or supplements currently being taken.

3. Temporary Prohibitions

Devarda.ink reserves the right to refuse service if you are in any of the following conditions:

  • Under the influence of alcohol or drugs.
  • Showing signs of intoxication or unusual behavior.
  • Having open wounds, irritation, or active infection in the area to be tattooed.
  • Having recently applied sunscreen, tanning products, or creams to the area.
  • Having undisclosed medical conditions or conditions that, in medical judgment, make tattooing inadvisable.

5. Photo Usage Rights (Photo Release)

Devarda.ink may use photos of your completed tattoo for the following purposes, unless you choose to opt out:

  • Devarda.ink website portfolio and social media.
  • Promotional and marketing materials.
  • Editorial publications and press.

You may decline photo usage by indicating so on this form. Declining will not affect the quality of service or the result of your tattoo.

Note: If you decline photo usage, Devarda.ink will respect your decision and will not display your tattoo photos in promotional materials or public portfolio.

6. Risks and Consequences

A tattoo is a procedure involving skin penetration. Associated risks include:

  • Local or systemic infection.
  • Allergic reaction to ink or aftercare products.
  • Scarring or keloids.
  • Changes in shape or color over time.
  • Reactivation of existing skin conditions.

You acknowledge that these risks have been explained to you and you accept these risks as part of the decision to receive tattoo services.

7. Consent Declaration

By signing this form, you declare that:

  1. The information you have provided is accurate and complete.
  2. You have read and understood the entire contents of this consent form.
  3. You consent to the tattoo service with full knowledge of the risks and consequences.
  4. You are at least 18 years of age.
